A PATIENT WITH DRUG ABUSE.
The patient has been a regular drug abuser and reports that he has been taking MDMA (ectasy) for the past three years. During this period, he has noticed that his general mental health has become impaired. This impairment includes a general confusion, insomnia, depersonalization and paranoid psychosis.
Studies have indeed shown that MDMA use is related to a lowering serotonin, which is associated with depression and anxiety. Severe chronic anxiety can be treated with diazepam, fluoxetine or sertraline,insomnia with temazepam and panic attacks with lorazepam. However, because all of these drugs are potentially addictive, the patient’s GP thought it best to use an antidepressant for no more than a few weeks.
Unfortunately the patient returned to using the drug whilst at a ‘rave’ where he took twice his normal intake of MDMA and was overwhelmed by the combined effect of drug and the mesmeric, trance-inducing music and psychedelic visuals. He reported to the first-aid facility for assistance where unfortunately he experienced a judgmental attitude from staff.

This was particularly unhelpful  because this young man has difficulties with authority figures and he became uncooperative and eventually refused the assistance that was offered.
The patient does not show the typical denial defence mechanisms of many MDMA users and there is a reason to believe that he will be able to successfully withdraw from the drug without the help of antipsychotics such as haloperidol.
